Fuel for vehicle:
Calculate the gallons used and your cost for them one of two ways: either

Vehicle miles in the quarter : Mileage of the car (mpg): Price of gas $ per gallon, or
Gallons bought during the quarter: at Price of gas $ per gallon, or
Total cost at the pump during the quarter: $

Then to find the self-tax, specify either

cents per gallon, or
percent of pump cost

Fuel for home heating: specify either

cents per gallon for the gallons of home oil used in the quarter, or
percent of home fuel cost, for your $ cost of fuel for the quarter (indicate whether fuel is gas or oil at per gallon )

Air travel: The government says the average is 42.6 person miles per gallon, though more if the plane is full,
and variable according to type of plane and other factors. This means that if you are the sole occupant of a car that
gets 42.6 mpg, flying and driving have equivalent carbon use. If you have a passenger, driving is better.
Specify either

cents per gallon of airline fuel for the miles you flew in the quarter, or
percent of your $ cost for the flights

Fuel to create electricity: One estimate is that it takes a gallon of fuel oil to produce 17 kWh delivered to a home.
Specify either

cents per gallon used to make the kWh of power you used, or
percent of your $ electricity cost for the quarter
